Assessment Process

Deciding on the best facility for your loved one can be a difficult and painful process. We’ve designed our assessment process to be as stress free as possible for you and for your loved one. We begin with an initial meeting with the family to discuss the patient’s needs and any concerns you might have. Next we’ll meet with the potential resident in their current environment. We’ve found that this will allow the patient to be more comfortable and remove any potential stress triggers. We’ll evaluate the patient’s physical abilities and mental capacities to determine if our facility can provide them with the safe care that they deserve.

Next, we’ll have a follow up meeting with the family to access the different options available. If it’s determined that AlzCare is a good fit for your loved one, we’ll coordinate with his/her doctors to make sure our care plan is tailored to his/her specific and unique needs.
